Students participating in any school sports program in the 2021-2022 school year are required to have a physical completed within the past 12 months on file at the school.  These physicals need to be completed before your child begins practice.  Physicals will be provided at Sumner Attendance Center on February 23rd.  There is no cost for the physicals.  The physical examinations will be provided by the Lawrence County Memorial Hospital Primary Care Clinic. The District offers these physicals as a service and parents are under no obligation to have their child’s physical done at school.  If preferred, you are welcome to have the physical completed by your child’s personal health care provider.  In order for your child to have their physical at school, you must complete the consent form and fill out the highlighted general information and health history section of the physical form. These can be picked up at the school office by you or your student.  If your child will be in 6th grade next year a physical form and consent have been mailed to your residence.  The consent form and the highlighted health history section of the physical form must be completed and returned to the school by February 16th.    Please feel free to call with any questions.
